Dr. Kellum highlighted that localized swelling in the feet and ankles is “rarely just a local problem,” noting that it typically reflects an underlying system—particularly the venous and lymphatic systems—becoming overwhelmed. He stressed that the body’s ability to manage fluid return from the lower extremities is heavily dependent on muscle activity and circulation dynamics.

A central point in his commentary is the role of the calf muscle as a physiological pump. He described it as the body’s “second heart,” explaining that each contraction assists in pushing venous blood and lymphatic fluid upward toward the core circulation. This mechanism is especially important given the challenges of gravity on the lower limbs.

Dr. Kellum also underscored the importance of frequent movement throughout the day as a primary preventive strategy. He advised that prolonged sedentary periods—particularly beyond 45 minutes—can actively impair venous and lymphatic return, increasing the likelihood of fluid pooling in the lower extremities. Regular walking and movement breaks were highlighted as essential interventions for individuals who sit or stand for extended periods, including office workers and frequent travelers.
